Nel 2005, ha iniziato ad esibirsi regolarmente al The Comedy Store di West Hollywood, California. Maniscalco cita Jerry Seinfeld, George Carlin, Brian Regan, John Ritter, Johnny Carson, Andrew Dice Clay, Bill Burr e Don Rickles come influenze comiche. Da allora, ha fatto una mezz’ora per Comedy Central Presents e cinque speciali di un’ora. Il suo primo speciale, Sebastian Live, è uscito il 2 giugno 2009. I suoi successivi tre speciali sono andati in onda su Showtime, con What’s Wrong with People? uscito il 5 gennaio 2012, Aren’t You Embarrassed?, registrato a Chicago, uscito il 14 novembre 2014, e Why Would You Do That?, registrato a New York City e uscito nel 2016.
Maniscalco è stato uno dei quattro comici presenti nel Wild West Comedy Show di Vince Vaughn: 30 Days & 30 Nights – Hollywood to the Heartland, insieme ad Ahmed Ahmed, John Caparulo, e Bret Ernst. Maniscalco ha partecipato a Best Night Ever di Russell Peters, Comedy Central Presents, The Late Late Show with Craig Ferguson, The Tonight Show with Jay Leno, The Jay Leno Show, The Tonight Show Starring Jimmy Fallon, Conan, e The Late Show with Stephen Colbert. Inoltre, è apparso in diversi film, come The Nut Job 2: Nutty by Nature, The House, Tag, e Cruise, e ha un podcast intitolato The Pete and Sebastian Show con il collega comico Pete Correale. A partire da gennaio 2017, hanno uno show sul canale 99 di Raw Dog Comedy della radio satellitare Sirius XM. Nel 2016, Maniscalco è apparso nella settima stagione di Comedians in Cars Getting Coffee. Ha pubblicato il suo libro di memorie Stay Hungry il 27 febbraio 2018. Uno speciale Netflix intitolato anch’esso Stay Hungry è stato rilasciato il 15 gennaio 2019.
Nel 2018, Maniscalco ha fatto il suo debutto cinematografico, nel ruolo di Johnny Venere nel film premio Oscar Green Book. Maniscalco ha ospitato gli MTV Video Music Awards 2019 il 26 agosto. Nel novembre 2019, Maniscalco è apparso in The Irishman di Martin Scorsese interpretando “Crazy” Joe Gallo accanto a Robert De Niro, Joe Pesci e Al Pacino. Ha anche ospitato il podcast di accompagnamento in tre parti per il film, pubblicato per la prima volta il 2 dicembre 2019, intitolato Behind the Irishman.
